IL18BP가 암호화하는 단백질은 전염증성 사이토카인(cytokine) IL18의 저해제(inhibitor)로 작용하여, IL18에 결합해 수용체(receptor)와의 결합을 차단하고 IL18 유도 IFN-gamma 생성을 억제함으로써 제1형 보조 T세포(T-helper type 1) 면역 반응을 감소시킨다. 이 단백질은 단핵세포(mononuclear cell)에서 지속적으로 발현·분비되며, Crohn병 환자의 장 조직에서 높은 수준으로 검출된다. 서로 다른 아이소폼(isoform)을 암호화하는 선택적 스플라이싱 전사체 변이들도 보고되었다.
